- Call the Connecticut Birth to Three system at 1-800-505-7000, or 211 and follow the prompts.
- Complete the Sparkler questionnaire
- Download the app Sparkler
- Use the code “Middletown” when prompted
- If your child is 2 years, 9 months or older:
- Call the Middletown Preschool Program for a screening
